Elevate Independent Facilitation's mission is to empower individuals with developmental disabilities to lead fulfilling and independent lives. We achieve this by combining progressive person-centered practices with compassionate and effective advocacy, helping our clients and their families navigate complex service systems, access vital resources, and coordinate the support they need to achieve their goals.
To support person-centered planning, we begin by understanding the individual’s current situation, including their unique strengths, abilities, and capacities. Next, we collaborate with clients to grasp their vision for the future, encompassing their hopes, dreams, and aspirations. We then identify any challenges and barriers they face in achieving these goals. While developing a plan to guide clients toward their ideal future state, we utilize all available generic services provided by state agencies, free services, and those covered under a self-determination plan. We outline a step-by-step plan to help clients leverage these services, including budgeting, advocacy, and approvals, ensuring they can effectively implement the plan.
Kelly Hoopes earned his certification as an Independent Facilitator from the California State Council on Developmental Disabilities. With personal connections to individuals benefiting from these services, Kelly recognizes the critical role of Self-Determination and Person-Centered Planning in enabling people of all abilities to lead enriched lives. Beyond his work as an Independent Facilitator, he co-founded a non-profit organization, the Self-Determination Tech Alliance (501c3), aimed at empowering adults of all abilities. This organization focuses on providing digital and entrepreneurial opportunities, with a strong commitment to promoting self-determination and self-advocacy.
